Determine the verb (what is acted upon the subject) in the active voice, then change the verb to the form: be + past participle of the verb.
(be-verbs: is, are, am; was, were)
Examples in different tenses of the verb:
Simple Present Tense
Active: Mary washes the dishes everyday.
Passive: The dishes is washed by May everyday.
Present Continuous:
Active: Mary is washing the dishes.
Passive: The dishes is being washed by Mary.
Simple Past:
Active: Mary washed the dishes.
Passive: The dishes were washed by Mary.
Past continuous:
Active: Mary was washing the dishes when her friend called.
Passive: The dishes was being washed by Mary when her friend called.

Present Perfect:
Active: Mary has washed those dishes in the cabinet.
Passive: Those dishes in the cabinet have been washed by Mary.
Present Perfect Continuous:
Active: Recently, May has been doing the research work.
Passive: Recently, the research work has been being done by Mary.
Past Perfect:
Active: Mary had done the research work recently.
Passive: Recently, the research work had been done by Mary.
Past Perfect Continuous:
Active: Mary had been preparing the research work for two months before she moved to Manila.
Passive: The research work had been being prepared by Mary for two months before she moved to Manila.
